Important Things to Know for 2012-2013

Plan Year September 1, 2012 to August 31, 2013

New Plan Changes are Effective 9/1/2012

  • NEW PPO Dental Insurance by Cigna: Plan allows participants the freedom to choose any dentist. Two options are offered a High Plan and Low Plan. Both plans offer increasing Calendar Year Maximums: Year 1 - $1,000, Year 2 - $1.250 and Year 3 and beyond $1,500+ and $50 deductible/$150 family max. Plan pays 50%Orthodontia with a lifetime maximum of $1,000 for dependent children to age 19. There are NO waiting Periods. All coverage begins September 1, 2012.

    High Option Plan: Plan pays 100% Preventative & Diagnostic Services, 80% Basic Restorative Services, and 50% Major Restorative Services. New Rates: EO $28.37; EE + Spouse $54.16; EE + Children $68.98 and EE + Family $94.80.

    Low Option Plan: Plan pays 80% Preventative & Diagnostic Services, 50% Basic Restorative Services, and 25% Major Restorative Services. New Rates: EO $15.84; EE + Spouse $30.22; EE + Children $38.49 and EE + Family $52.89.

  • Long-Term Disability Protection by Unum: Plan provides a monthly income to an individual that is disabled due to an accident or illness. There are 2 different plans available with benefits becoming available from the 1st day of disability to as late as the 180th day. Benefits are payable up to 2 years or to age 65 for injury and sickness depending on which plan you choose. **All new or increases in coverage are subject to pre-existing condition exclusion.** Unum has agreed to lower the Educator DisabilityRates by 7% and also add the Medical Treatment Benefit back effective September 1, 2012. The Medical Treatment Benefit reimburses $50.00 for Illness or $100.00 for Accident up to four times a year for those employees covered under the Disability Plan.

  • NEW Cancer Insurance by Loyal American: The new cancer coverage offers three options to everyemployee in the Coop. All three options offer richer benefits and very competitive pricing compared to the current plan. Radiation/Chemotherapy benefits range from $7,500 annually to $20,000 annually. All options include $50 Wellness benefit and a very rich first occurrence benefit ranging from $3,000 to $6,000. All three options also offer a much richer optional ICU coverage.   • NEW Basic & Term Life/AD&D Insurance by Unum:(Coverage amounts vary by district). Voluntary Term Life/AD&D Plan allows enrollment on a Guarantee Issue Basis (no health questions asked) and is available as an age-banded rate plan that allows benefits up to $500,000, based on salary, for the employee; coverage is also available for spouse and dependent children. This year only Unum has agreed to allow every employee and spouse to elect coverage or increase their coverage up to the Guarantee Issue Amount without answering any health questions. Employee Guarantee Issue amount is $200,000 or max 7 times salary and spouse if Guarantee Issue up to $50,000.

  • Medical & Dependent Care Accounts by National Benefit Services: (NBS) Tax-sheltered flexible spending accounts allow an individual to set aside dollars to pay for future health care and dependent care expenses. Eligible expenses must be incurred within the plan year and contributions are a use it or lose it. Flex Cards will be offered to healthcare reimbursement participants at “NO COST”. If you currently participate in healthcare Reimbursement and have a Flex Card, please DO NOT throw your flex card away. NBS will reload your current flex card with your with new 2012-13 contribution. Direct deposit is available. The medical reimbursement annual maximum is $2,500 / plan year and the dependent care reimbursement annual maximum is $5,000 / plan year, if married filing jointly and $2,500 / plan year, if filing single. **Please remember to keep your receipts!** As a result of the Health Care Reform Act, the maximum annual contribution for your Medical Reimbursement will be $2,500 starting September 1, 2012.
