Important Things to Know for 2011-2012

  1. NEW – NBS will be the Third Party Administrator (TPA) for WTXEBC for plan year September 1, 2011 through August 31, 2012. A NEW Visa Card will be issued to those districts who participate in the flex card plan. Direct deposit is available. Please remember to keep your receipts if you use your Visa Flex Card. The medical reimbursement annual maximum is $4,000 per year and dependent care reimbursement annual maximum is $5,000/ plan year for those married filing jointly and $2,500/plan year filing single. If you are using the Visa Flex Card, please be sure to hold on to your receipts for 12 months.


  2. Dental Plan - Dearborn National will remain the dental carrier for WTXEBC. Dearborn National offers two plans: a high and a low option dental plan. Both plan options offer a passive PPO which allows participants the freedom to choose any dentist. There are no waiting periods on either plan. Please refer to the plan brochure under Benefits and Forms for more information.


  3. Critical Illness Plan – Assurant will remain as the Critical Illness carrier for WTXEBC. Critical Illness coverage is designed to pay a lump sum benefit in the event you suffer from a heart attack, stroke, coronary bypass surgery, blindness, major organ failure, end stage kidney disease, paralysis or coma. Offered on a guaranteed issue basis.


  4. Identity Theft Protection Plan - ID Watchdog will remain the carrier for WTXEBC. ID Watchdog monitors all three credit reporting agencies and helps prevent your identity from getting stolen. ID Watchdog has a patent-pending Fraud-monitoring technology that provides you with easy to read monthly reporting alerts to any identity threats.


  5. Voluntary Group Term Life - Dearborn National will be the group term life provider for WTXEBC. New employees and districts will have a Guaranteed Issue amount without having to answer medical questions. Employee coverage will be guaranteed issued up to $200,000, spouse up to $50,000 and child(ren) up to $10,000. Existing employees that elect the plan will have to complete Evidence of Insurability application before coverage is active.


  6. Vision Plan – Superior Vision will remain the vision carrier for WTXEBC. Members pay a co-pay for the in-network benefits, exam co-pay $10.00 and material co-pay $25.00. Out-of-network vision services are reimbursed up to a certain dollar amount for covered expenses. Exam, lenses and frames (with plan allowances) are covered in-network once every 12 months.


  7. Long Term Disability Plan – Unum will remain the long term disability carrier for WTXEBC. Plan provides a monthly income to an individual that is disabled due to an accident or illness. There are 2 different plans available with benefits becoming available from the 1st day of disability to as late as the 180th day. Benefits are payable up to 2 years or to age 65 for injury and sickness depending on which plan you choose. All new or increases in coverage are subject to pre-existing condition exclusion.


  8. Cancer Plan – American Public Life will remain the cancer provider for WTXEBC. The plan allows for enrollment on a guaranteed issue basis (NO health questions asked) however, no benefits are payable during the 1st year of coverage for a pre-existing condition. Cancer insurance is designed to be a supplement and pays for many cost not covered by your major medical plan. There are 2 plan options available, both offer a lump sum Critical Illness benefit and reimburse up to $50.00/annually for each insured person for screening test results.


  9. Accident Plan – American Public Life will remain the accident provider for WTXEBC. The plan pays benefit amounts for covered medical expenses as a result from an accident, directly to you! Coverage is available for ages18-64 and is portable. You can choose to keep your benefit even if you leave the district or retire.


  10. Individual Permanent Life Insurance – The plan provides a guaranteed death benefit to age 121 and level premium that guarantees coverage for a signification period of time. Employees can insure their spouse, children or even grandchildren without having to insure coverage on themselves.


  11. WTXEBC Benefit Plans for 2011-2012 plan year include Dearborn National Dental, Superior Vision, APL Accident, APL Cancer, Unum Select Disability, Dearborn National Voluntary Group Term Life and AD&D, Texas Individual Life, Assurant Critical Illness and ID Watchdog Identity Theft Protection. New products include: NBS Flexible Spending Accounts.
